Social Worker - Social Services

The Staff Social Worker reports directly to the LCSW and assists in meeting the psychological needs of patients to utilize medical care in order to achieve their optimal level of health. 



Job Responsibilities/Duties

  • Interviews patients and significant others to obtain information about social, economic, emotional and family situation. 
  • ​Interprets social and family information to attending physicians and other patient care team members. 
  • ​Determines social assessments for discharge planning.
  • ​Participates as a team member in conferences on behalf of patients with other health care personnel and other community agencies. 
  • ​Enables patients and their significant others to understand, accept and follow medical recommendations through individual or group conferences.
  • ​Contacts community resources and other pertinent sources in order to mobilize environmental services in patient's behalf. 
  • ​Performs a variety of services such advising on social problems, arranging for discharge of post-hospital plans, placement of patients in skilled nursing facilities or board and care homes, financial assistance, transportation. 
  • Develops and maintain good working relationships with other hospital and medical staff as well as community health, welfare and social agencies.
  • ​Participates and assists in departmental studies and projects as assigned. 
  • Performs basic administrative tasks related to job. 
  • Maintains the confidentiality of all information. 
  • Provides patient and families with counseling services.


Qualifications

Minimum Education: A Bachelor's Degree in Social Work or related field required.

Minimum Experience: Minimum of one (1) year experience in a hospital social services setting, preferred.

Req. Certification/Licensure: None.
