Activity Coordinator - Sub-Acute

The Activity Coordinator reports directly to the Resident Assessment Supervisor (RAS). Activity Coordinator assesses, plans, implements and evaluates programs for patient activities which are multi-faceted, meet the patient’s functional needs, and reflect interests of each patient.

Job Responsibilities/Duties

  • Cooperates and consults with appropriate staff members regarding residents.
  • Consults with the Clinical Director to determine the best application of services.
  • Provides room visits to residents who are unable or unwilling to attend group activities.
  • Transports residents from their rooms to the activity room and back.
  • Maintains current documentation in the Medical Records of the residents.


Minimum Education: High School diploma or equivalent.

Minimum Experience: Two (2) years experience in a social or recreational program within the last five (5) years. One of which was full time in a patient activities program in a healthcare setting, preferred.

Req. Certification/Licensure: Activities Director Certificate; American Heart Association (BLS) within six (6) months of hire; California Certified Nursing Assistant preferred.
