RN Care Coordinator

The  Registered Nurse (RN) Care Coordinator is responsible for meeting the medical appointment needs of the residents including, tracking of, obtaining authorization for, scheduling of and coordinating the appointment of residents to off site appointments.

Job Responsibilities/Duties

  • Responsible for communication of outcomes of appointments to MD's and families etc.
  • Coordinates the completion of the Minimum Data Sets (MDS) timely and accurately by the Interdisciplinary Team. 
  • Assist the Nursing Director in Performance Improvement data collection, maintain required logs (census / dental / immunization etc) and attend the Interdisciplinary Team Meeting.
  • Promote timely access to appropriate care.
  • Increase utilization of preventative care.
  • Reduce emergency room utilization and hospital re-admissions.
  • Increase comprehension through culturally and linguistically appropriate education.
  • Create and promote adherence to a care plan, developed in coordination with the patient, primary care provider, and family/caregiver(s).
  • Increase continuity of care by managing relationships with tertiary care providers, transitions-in-care, and referrals.
  • Increase patients’ ability for self-management and shared decision-making.
  • Provide medication reconciliation.
  • Connect patients to relevant community resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs.


Minimum Education: Registered Nurse or individual with Bacclaureate degree in social work.

Minimum Experience: Minimum Two (2) years of experience preferred.

Req. Certification/Licensure: California RN in good standing for RN's. Bachelors degree for others.
